Typically, as you first become eligible for Medicare, you are guaranteed access to a Medigap plan.  However, there are strict rules and qualifying for later entry or re-entry.

Certain Medigap Plans offer benefits that some enrollees prefer (over Medicare Advantage):

  • No networks, you can choose almost any Doctor

  • No referrals may allow faster access to specialists

  • Less 'step care' treatments, and less pre-approvals

  • Substantial International non-emergency care

  • No copays for Emergency Room or Drs Offices

  • Very small 'out of pocket' amounts

  • Very reduced 'cancer copays', if any​

Medicare Supplement Plans (Medigap)

Protection From Gaps in Orig. Medicare A & B

Medicare Supplement (or Medigap) plans provide insurance coverage for the 20% expense "gaps" that 'Original Medicare' Parts A & B do not cover, including deductibles. co-payments and co-insurance.

Qualifications

You must qualify for the Part A of 'Original Medicare' (ask Jeff) and be enrolled for Part B and pay the Part B monthly premium, including any Income Related Monthly Adjustment Amount (IRMAA) increase.

 

Any Doctor, No Networks, No Referrals Needed

With a Medicare Supplement plan, you can utilize any doctor / provider that accepts Medicare.  No referrals are needed as long as you utilize a doctor or provider that accepts Medicare (which is the vast majority of doctors and providers).

 

Portable Coverage, Accepted Anywhere

Medicare Supplement plans cover the entire United States and you can rely on uninterrupted coverage when you travel or relocate.

Specialty / Premier Medical Facility Acceptance

Google says "Johns Hopkins Medicine hospitals and most Johns Hopkins University physicians generally accept Original Medicare, but they do not have contracts with most Medicare Advantage Plans."

 

Robust International Coverage

Medicare Supplement plan also substantial insurance coverage if you travel internationally, and not just for emergency-related medical services.

'Medicare Advantage' Plans (Part C)

Medicare Advantage Plans

An alternative privatized version of Medicare coverage where Medicare-approved insurance carriers are paid a fee by the Centers for Medicare and Medicaid Services (CMS) to provide a package of healthcare and related services following guidelines set by Medicare.  Medicare Advantage plans are often called "Part C" of Medicare (or "MA" or "Advantage" plans).

 

Qualifications

You must qualify for the Part A of 'Original Medicare' (ask Jeff) and be enrolled for Part B and pay the Part B monthly premium, including any Income Related Monthly Adjustment Amount (IRMAA) increase.


Low Monthly Premiums

Medicare Advantage plans may offer low monthly premiums. They typically offer prescription drug coverage, and may include added benefits such as vision exams, dental cleanings, and more.

Provider Networks May Be Limiting

In many cases, you’ll need to use health care providers who participate in the plan’s network and service area for the lowest plan costs. These plans set a Maximum Out Of Pocket (MOOP) each year for covered services, to help reduce your overall expense, however an annual MOOP can be $7500+

Plus, Google says "For cancer-related services under Medicare Advantage, you will typically pay a copay, which is a fixed amount, or a coinsurance, which is a percentage of the total cost, for each service. Many plans require a coinsurance of 20% for cancer treatments, such as chemotherapy and radiation, until you reach the plan's out-of-pocket maximum. Once you reach the maximum, the plan will pay 100% of your covered costs for the remainder "of the year. 

Adding a Prescription
Drug Plan (Part D)

Medicare Part D Prescription Drugs
Medicare Part D covers the cost of many prescription drugs. You'll secure Part D coverage through private insurance companies (who partner with Medicare), and the formulary of prescription drugs that your plan covers will vary by provider within overall guidelines set by Medicare.

Brand-Name and Generic
Most Part D plans provide both brand-name and generic prescription drug coverage. At a minimum, every provider must cover 2 options within the most common drug categories, meaning that Plan D will provide drug coverage for the majority of conditions.

2 Ways to Access Part D / Prescription Drug Plans
1) You can choose from a variety of Part D offerings from leading insurance companies that may cover the majority of your prescription costs each year.

 

1) A Part D plan is typically added to a Medicare Supplement plan.  Part D plans typically have a modest monthly premium and an annual MOOP (maximum out of pocket) of $2000 as of 2025.

 

2) The second way to have prescription drug coverage is a Medicare Advantage plan which bundles in prescription drug coverage (an MAPD plan). Consumers should confirm any MAPD plan they consider has their required or anticipated prescriptions. An annual MOOP (maximum out of pocket) of $2000 typically applies as of 2025.
